Personal Information Updates

Active Students:

• Name Change Form
• Social Security Number Change Form
• Date of Birth Change Form
• You can update your address and phone number through your MyJaxState account.
  1. Login to MyJaxState.
  2. On the Student Card, select More.
  3. Scroll down and select Update Addresses and Phones.
  4. Use the pencil icon to edit addresses or phone numbers. 

  If you are accessing MyJaxState on a phone or another device with a small screen, selecting Update Addresses and Phones will open your Personal Information page but your full profile will not be viewable.

  1. Scroll to the bottom of the page.
  2. Select View Full Profile in the gray bar.
  3. Choose Phone Numbers or Addresses to make your updates.

Inactive Students/Alumni: Please print out the Personal Information Update form and fax, mail, or scan & email the form with supporting documentation.
