PEEHIP to Offer Flu Shots to JSU Employees on October 15

Healthwise, your Public Education Employees Wellness Program will be on campus giving FLU SHOTS to JSU employees on Wednesday, October 15, 2014, from 9:00 am to 3:00 pm at The TMB AUDITORIUM. There is no cost to you or your dependents if you have the PEEHIP medical coverage, PEEHIP supplemental coverage or one of the Southland optional plans (dental, cancer, hospital indemnity or vision). The Humana/Compbenefits plan is NOT an eligible plan. Employees/Retirees will be required to show proof of insurance before the vaccination can be given (no exceptions will be made).

Appointments are not necessary and vaccinations will be given on a FIRST COME FIRST SERVE BASIS. In order to avoid delays, we suggest you complete the attached vaccination form in advance and present it at the time of vaccination along with your proof of coverage (listed above). Please remember to wear clothing that will expose your upper arm where the vaccination will be injected.

Also, a limited number of Health Screenings will be given on a first come first serve basis.
