JSU Forms and Deadlines

There are several JSU forms that students intending to participate in the study abroad program are required to complete. 

IMPORTANT NOTICE: JSU's International Travel Risk Committee will need to review the destinations designated as Level 3 and Level 4 by the Department of State no later than 45 days before the travel date. Participants-students and faculty leaders are responsible to understand the cancellation and refund policies of the providers. In the event the program gets canceled for the safety and health of the participants, the participants will be responsible for any financial losses. Email us at studyabroad@jsu.edu if you need a form to make an appeal to travel to a restricted destination. Restricted destinations are the destinations designated as Level 3 or Level 4 by the Department of State. 

All participants will need to complete the COVID-19 Acknowledgement Form.

A. All students will need to complete the following forms: 

1. Conduct Inquiry Form 

2. JSU Study Abroad Application Form 

3. Financial Aid Form 

4. COVID-19 Acknowledgement Form

B. Students participating in the Direct Exchange Study Abroad program or Non-Affiliate Study Abroad Program or CCSA Study Abroad Program will be required to complete additional form listed below: 

1. JSU Transient Form  

C. All students who need to report medical conditions will need to complete the form listed below:

D. All students who need to request accommodation will need to complete the form listed below:
