Document Delivery Request Form

NOTE: Document Delivery Services are available to JSU Distance Education students and faculty who reside outside Calhoun County and in an area where no formal agreement exits between JSU and the local library to support course offerings. Printed materials owned by the Houston Cole Library will be loaned and sent to JSU distance education students and faculty. Requests should be limited to JSU distance education course related research. JSU students enrolled in courses at the Gadsden State Community College branch should request document delivery through the Meadows Learning Resource Center. Please do not phone requests. Questions about the library's distance education services should be directed to Yingqi TangTo request books, documents, or photocopies of articles that we do carry, use the following form. Required fields are indicated with an asterisk (*). Make sure you read the Copyright Restrictions section prior to submitting your request.
