JSU Employee Flu Shots Will Be Given Away On October 17

Healthwise, JSU's Public Education Employees Wellness Program will be on campus giving Flu shots Thursday, October 17, 2013, from 9:00 am to 3:00 pm at the TMB Auditorium. There is no cost to JSU full-time faculty and staff employees or their adult dependents (must be 18 or older), if the employee has the PEEHIP medical coverage, PEEHIP supplemental coverage or one of the Southland optional plans (dental, cancer, hospital indemnity, or vision).  The Humana/Compbenefits plan is NOT an eligible plan.  Employees will be required to show proof of insurance before the vaccination can be given (no exceptions will be made).

Appointments are not necessary and vaccinations will be given on a FIRST COME FIRST SERVE BASIS. In order to avoid delays, it is suggested that employees complete the vaccination form in advance and present it at the time of vaccination along with your proof of coverage (listed above).  The vaccination form has been revised and is a bit different than the vaccination form completed in the past.  Please review the form carefully and complete all the necessary sections.  Also, please remember to wear clothing that will expose the upper arm where the vaccination will be injected.
